Growth Hormone and Insulin-Like Factor I in Chronic Renal Failure

Abstract

Growth hormone (GH) response to pharmacological stimuli and insulin-like factor I (IGF-I) levels were studied in 10 patients with chronic renal disease. Basal GH levels were normal (4.58±3.54 ng/ml) in all patients when compared with control subjects (4.67±2.83 ng/ml). GH response to L-dopa stimulation was also normal in these patients when compared with the results of control subjects. Height SDS was -3.82±0.54 in prepubertal children with chronic renal disease and -0.35±0.74 in control subjects. There was a significant difference (p<0.001) in height SDS between these two groups; a severe growth retardation was present in our patients. The mean IGF-I level was 79.6±20.79 ng/ml in prepubertal children with chronic renal disease and 136.5±61.04 ng/ml in control subjects. IGF-I levels were low in prepubertal patients when compared with the control subjects but this difference was not significant. In conclusion, a severe growth retardation was present in our patients with chronic renal disease although serum growth hormone levels were found normal and the response to L-dopa stimulation was also normal. A decrease in insulin-like growth factor I was also observed in these patients.
